Birmingham Direct Cataract Referral with Choice of Provider

For use for patients whose GP is associated with:

Birmingham Cross City CCG

Birmingham South Central CCG

Solihull CCG

In these areas IOP Repeat Readings can be reported and claimed via Optoserve provided by Webstar Health.

Cataract fees can be claimed via Optoserve but the referral form must still be faxed to the relevant provider.

N.B. Areas where a new LOCSU pathway based community service has been established (currently only Wolverhampton) should report and claim via Optoserve provided by Webstar Health
