The enhanced services in this section are open to Optometric practices who have GOS contracts in Western Cheshire and applies ONLY to patients who are registered with a GP practice in Western Cheshire . If a patient is registered with a GP in a different area (e.g. Central Cheshire, Stockport, North Staffs etc) then these services may not be used for that patient. You would need to check with the patient's area to see what their arrangements are for such services, but it is unlikely that you will be able to provide enhanced services for "out of area" patients as a general rule.
NHS Western Cheshire uses a new ophthalmic referral form ( ORF ) and recommends that ophthalmic practitioners use this form instead of a GOS 18 for ophthalmic referrals to GP's in Western Cheshire. **************Minor Eye Conditions - Complaint Form*****************
At a Western Cheshire Eye Health Group meeting recently, the CCG have asked for ALL concerns about the Minor Eye Conditions Service to be reported on the attached form, however trivial.
They are hoping to gather more evidence on the performance of the contract. It would also be useful if Optometrists could report any patients not referred to this scheme if they think service/treatment would be better elsewhere.
If you are unhappy with the service this is the way to demonstrate to the commissioners that the service is underperforming so please take the time to fill out the form.
